Bot flys. yecch. I has done me a bit of research on these critters. It's disgusting, but extremely interesting.
The mama Bot Fly lays her eggs on a ordinary mosquito. When the skeeter bites a potential host, the eggs get stashed on the surface of the host's skin. The host's body heat excites the eggs into hatching, whereupon a lucky pupa bails for the hole left by the skeeter bite. That is what they are extracting from these people. A pupal Bot fly.

Bot fly was my first though when I saw it, though they are usually a bit smaller. A friend's cat had one between her shoulder blades.
And they do have them in other parts of the US - north or northeast, I think.
Gross. If you cover up the hole with a vaseline-covered piece of guaze, you can get it to come out a bit to breathe.

Apparently, they have little spikey dealios that encircle their bodies which are situated to make extraction difficult since they act like fishhook barbs.
