The language is NSFW.

I have no idea WTF the thing is they eventually extract from this guy's leg. Are there sub-cutaneous leeches?

Bot flys. yecch. I has done me a bit of research on these critters. It's disgusting, but extremely interesting.

The mama Bot Fly lays her eggs on a ordinary mosquito. When the skeeter bites a potential host, the eggs get stashed on the surface of the host's skin. The host's body heat excites the eggs into hatching, whereupon a lucky pupa bails for the hole left by the skeeter bite. There, it sets up housekeeping and continues to pupate until, if left alone, it will emerge a fully developed Bot fly.

That is what they are extracting from these people. A pupal Bot fly.

Bot fly was my first though when I saw it, though they are usually a bit smaller. A friend's cat had one between her shoulder blades.
And they do have them in other parts of the US - north or northeast, I think.
Gross. If you cover up the hole with a vaseline-covered piece of guaze, you can get it to come out a bit to breathe.

And that is when you can grab them with the tweezers and get to yankin'!

Apparently, they have little spikey dealios that encircle their bodies which are situated to make extraction difficult since they act like fishhook barbs.
