Thankfully is was my OK return. What do I need to do? Should I mail the OTC another copy with a note inside?

I'm 90% sure it was my OK return and not my feredal return.

PMITA state prison is probably better than PMITA federal prison. Maybe.

Call them and let them know what happened. They'd rather have you on the outside making money and being their bitch than on the inside being someone's bitch with no taxable income.

The Federal will return it to you with instructions on how to correct it before re-filing (i.e.-sign it). They won't accept it as filed until it is signed by the appropriate party. Not sure what the OTC will do, if anything, but it should be comparable.

Happened to me once. They (IRS, so OTC may be different) sent me a form that I had to sign and send back in. I did so and never heard another peep. You could also send a second, signed, copy in.
