there are a lot of programs out there with the ability to read the SMART info on your disk. this is one, NHC is another...and there are lots more. Linux boxes typically include a SMART reader on installation.

They are however limited. They only do a good job of monitoring that SMART info that your drive chooses to fill. Some manufacturers do not choose to fill all of the SMART blocks. But yeah, its a good idea to check this stuff from time to time. They can tell you when the drive is getting to the point that you should probably start considering a replacement.
