They cauterize the hole when they make it, just like they do when they do surgery on people, not much blood. The risk I'd think is of infection, given the open hole. But then it opens into the stomach with lots more bacteria in there, so maybe it's not so bad. I dunno.

FWIW, cows have more than one stomach.

And if you go through their rectum, you ain't gonna reach their stomach/s. I've been shoulder deep, and was still about 76 feet of intestine short of their stomach/s.
