Just a question: Why would any game NOT be in HD? Is it a shortage of cameras? What? And how many more years before the "LD" cameras are phased out?

It is definitely an equipment issue.

ABC seems to lag for no reason. They only seem to show their main game in HD.
FOX Sports Net is probably more of a cash flow issue, but they are working on it.
CBS seems to broadcast all their games in HD.

My guess is that they are all adding equipment every year and will probably all be completely converted in the next 5 years or so.

It is more of a HD production truck issue than it is a camera issue. The cameras are cheap compared to the production truck.

ABC is also ESPN, in fact ESPN now runs what used to be ABC Sports. They have games in HD everywhere, almost every night of the week. It seems that all national games are broadcast in HD while regional games are left to however many trucks are available.

Not all FSN games are in HD but many are. They typically do not do as many games as ABC/ESPN. I noticed fewer HD games when baseball season was still going on, with the season over the RSN's have another pair of trucks available (one home/one away truck) to cover football. I think Fox Sports and the Fox Regional Sports Networks pretty much operate completely independent of each other so there seems to be very little sharing of resources.

CBS does one, maybe two college games most weekends, this week they have three. They still have one or two NFL games in SD almost every weekend, depending upon the split with Fox. It is still much better than last year when half of their NFL games were still in SD. Next year I think they have to have all of them in HD per the contract with the NFL.

Here are all of the games for the three main networks this weekend.
